o   Who do you serve?

The Sulzbacher Clinics provide health services for the homeless, uninsured and those whose income falls below 200% of current Federal Poverty Guidelines in downtown Jacksonville and those within the 32233, 32250, 32082, 32224 and 32266 area codes of Jacksonville Beach.

o   Where are your clinics located?

The Sulzbacher Downtown Clinic is located at 611 East Adams Street, Jacksonville FL. The Sulzbacher Beaches Clinic is located at 850 6th Avenue South, Suite 500, Jacksonville Beach, FL.

o   What programs do you offer?

The Sulzbacher Clinics offer primary health, oral health and behavioral health services, including substance abuse counseling (in conjunction with Gateway). We provide an integrated approach to ensure the highest quality of comprehensive care for our patients. We also operate an onsite pharmacy, and participate in the various Prescription Assistance Programs offered by drug companies.

o   Who owns the clinics?

The Sulzbacher Clinics are part of the Sulzbacher Center, Northeast Florida’s largest provider of comprehensive services for homeless men, women and children, and one of only a few centers in the nation to offer a full range of services for the homeless 24 hours a day, 7 days a week, 365 days a year.

o   What are your clinic hours?

The hours for the Sulzbacher Downtown Clinic are listed below. Walk-ins are accepted on a first-come first-served basis.

§  Primary Care: Monday and Thursday 7:30AM – 5:00 PM; Tuesday and Wednesday 7:30AM-8PM; Friday 8:30AM – 3PM. Closed daily 12:00-1:00PM.

§  Dental: Monday and Thursday 7:30AM – 5:00 PM; Tuesday and Wednesday 7:30AM-8PM; Friday 8:30AM – 3PM. Closed daily 12:00-1:00PM.

§  Behavioral Health: Monday – Friday 8:00 – 4:30. Closed daily from 12:00 – 1:30.

The Sulzbacher Beaches Clinic is open Monday – Tuesday and Thursday-Friday 8:00AM – 4:30 PM, and Wednesday 8:00AM – 7:00PM. It is closed daily from 12:00 – 1:00PM. Walk-ins are accepted Monday & Thursday 8:00AM – 12:00PM. Please note that patients typically arrive around 7:00AM and will be seen on a first-come first-served basis.

o   Do you have after-hour or emergency clinics?

The Sulzbacher Clinics do not operate after hours, except for selected specialty clinics which are typically offered once a week. Patients who are experiencing a true emergency should go to the closest available hospital emergency room.

o   Do you accept walk-ins?

Walk-ins are accepted at both Sulzbacher Clinics. However they are only accepted Monday and Thursday 8:00AM – 12:00PM at the Sulzbacher Beaches Clinic. Same-day appointments are also available. 

o   Do you accept private insurance?

We accept private insurance, Medicaid and Medicare. Patients enrolled in these programs should bring their card to their first clinic appointment.

o   How do I make an appointment?

For an appointment at the Sulzbacher Downtown Clinic, please call 904-394-8069. For an appointment at the Sulzbacher Beaches Clinic, please call 904-224-3550.

o   What should I bring for my first visit?

We offer services on a sliding fee discount and no individual will be turned away for their ineligibility to pay. To qualify for the sliding fee discount, please bring identification – driver’s licenses, state-issued photo IDs or birth certificates are accepted. We also require proof of household income - pay stubs, bank statements, unemployment letters (including income) and Social Security letters (including income) are accepted. Please bring your Medicaid or Medicare card if you are currently enrolled in these programs.
